Payment Policy

Payment is due at the time services are rendered unless other arrangements have been made in advance. Please remember that you are fully responsible for all fees charged by this office, regardless of your insurance coverage.

Insurance & Billing

We will send you a monthly statement for your records. Most insurance companies respond within four to six weeks. If your statement does not reflect insurance payment within that timeframe, please contact our office.

Any remaining balance after insurance has paid is the patient’s responsibility. Prompt payment is appreciated. If needed, monthly payment arrangements must be made prior to the scheduled procedure.
